CJC-1295 DAC is a synthetic 30 amino acid analog of growth hormone releasing hormone (GHRH 1-29) modified with four amino acid substitutions (D-Ala2, Gln8, Ala15, Leu27) to resist enzymatic degradation, plus a maleimidopropionyl linker (the Drug Affinity Complex, or DAC) that forms a covalent bond with plasma albumin via cysteine 34. The albumin tether extends the circulating half-life from minutes (native GHRH and non-DAC modified GRF 1-29) to several days, producing sustained stimulation of pituitary somatotrophs through the GHRH receptor and elevating both growth hormone (GH) and insulin-like growth factor 1 (IGF-1). Pulsatile endogenous GH release is preserved rather than suppressed.
Ipamorelin is a synthetic pentapeptide (Aib-His-D-2-Nal-D-Phe-Lys-NH2) developed by Novo Nordisk that acts as a selective agonist at the growth hormone secretagogue receptor (GHSR-1a), the ghrelin receptor. It stimulates pituitary release of growth hormone with potency comparable to GHRP-6, but in published preclinical and human studies it did not significantly raise ACTH, cortisol, prolactin, FSH, LH, or TSH even at doses far above those producing maximal GH release. Unlike ghrelin itself, ipamorelin in published human studies did not produce a clinically meaningful appetite or food intake signal in healthy volunteers at the doses studied.
Tesamorelin is a stabilized 44 amino acid analog of human growth hormone-releasing hormone (GHRH(1-44)) with an N-terminal trans-3-hexenoyl modification that protects it from dipeptidyl peptidase-IV degradation. It binds the pituitary GHRH receptor and stimulates pulsatile endogenous growth hormone release, raising IGF-1 within physiologic ranges. Tesamorelin preserves the natural negative feedback of the GH/IGF-1 axis and does not act directly on adipose tissue; observed reductions in visceral adipose tissue are mediated through GH-driven lipolysis. It is FDA-approved (Egrifta, Egrifta SV, Egrifta WR) for reduction of excess abdominal fat in HIV-infected adults with lipodystrophy.
